About 2.30pm, on Friday, May 19, emergency services were called to the Golden Highway near the intersection of Ulan Road, Cassilis, following reports of a crash.
Initial investigations suggest a 4WD lost control and rolled on the highway.
One of the five occupants of the car, a young child, died at the scene.
The four remaining occupants have all been treated at the scene by NSW Ambulance paramedics for before being taken to Dubbo Hospital.
A crime scene has been established and investigations are continuing.
